Birth Amendment

Overview

Birth Amendment service allows parents to make specific changes to their child's birth record.

This service applies for:

  1. Changing a child’s name up until their first birthday.
  2. Changing the father’s details, including adding, removing, or correcting information related to the father on the official record.

By providing this birth amendment option, the VSU ensures that families have the ability to accurately update crucial details in their child’s documentation, reflecting any necessary revisions within the first year of life.

Process

Who can apply?

  1. Mother
  2. If both parents are listed on the record, then either Mother or Father can apply.
  3. If the mother is deceased – the father, grandmother, sister, aunt, etc. can register but need to provide the death certificate of the deceased mother.

Required documents

  1. Mother’s Identification document
  2. Father’s Identification document – if the father’s details are to be added
  3. Father’s consent form- if Father’s details are to be added
  4. Court order- in special cases such as removing the Father’s details.
  5. Mother’s Death Certificate- if Mother is deceased
